About The Recognition Night

The Entertainment Committee of MAC Youth  London will be hosting an awards ceremony to recognize outstanding individuals who have made a positive contribution to their community.  Help us highlight their accomplishments by nominating someone you know!

Do you know anyone that has any outstanding qualities or achievements? 

 If so, then you may want to consider nominating them for the upcoming MAC Achievement Awards, taking place on Saturday October 9, 2004 at 7:00pm.

One winner will be selected among all nominations, and will be judged by a panel of leaders within the community.  Anyone in high school, college and university in the London area, with outstanding achievement can be nominated. 

Nomination forms are due on Friday October 1, 2004 and you can email your nomination to macforms@yahoo.ca or fill an online nomination form at www.mac-forms.tk
